Position Overview
The LCSW plays a vital role in the primary care treatment team within the Rural Health Clinic, identifying, triaging, and managing patients with medical and behavioral health conditions. This role involves skill training, psycho-education strategies, and patient-centered interventions that are problem-focused and goal-oriented.

The LCSW applies advanced clinical skills to prevent, assess, and treat mental, emotional, behavioral, and addictive disorders. Responsibilities also include engaging in multidisciplinary treatment planning, providing patient and family psychotherapy, and collaborating with care management teams to support patients in Behavioral Health Integration and Community Health Integration programs.

This position requires flexibility, adaptability, and a commitment to delivering evidence-based, culturally sensitive best practices in a fast-paced, nontraditional clinical setting.


Key Responsibilities

  • Conduct patient interviews, assessments, and develop patient care plans.
  • Screen, assess, and treat behavioral health conditions (e.g., depression, anxiety, substance use disorders).
  • Utilize standardized screening tools such as PHQ-9, GAD-7, and SBIRT.
  • Contribute to team-based care for chronic disease management (e.g., diabetes, hypertension, heart disease, obesity).
  • Identify at-risk patients and develop preventative care plans.
  • Perform Social Determinants of Health Risk Assessments, Psychiatric Diagnostic Evaluations, and provide Individual and Family Psychotherapy.
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care.
  • Utilize evidence-based psychosocial treatments for mental health and substance use disorders.
  • Document in Electronic Health Records (EHR) accurately and in a timely manner.
  • Monitor individual and population health outcomes to improve patient care.
  • Connect patients and families with helpful community resources.
  • Adapt services to language and cultural preferences, ensuring inclusivity and diversity.
  • Stay current on federal, state, and local healthcare regulations and ensure compliance.
  • Utilize telehealth technology to support remote patient care and education.

Qualifications
Required:

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • 2+ years of clinical social work experience.
  • Active LCSW license in the State of Wyoming (in good standing).

Preferred:

  • Experience in a medical setting, Federally Qualified Health Center (FQHC), or Rural Health Clinic (RHC).
  • Experience working with diverse patient populations.
  • Familiarity with integrated behavioral health models.
